Role of service/pet dogs: People with disabilities

As more people increase their activities with dogs, we at the School of Veterinary Medicine at the University of California, Davis, are gathering information to learn more about the role of dogs in the lives of people with disabilities.

This questionnaire is designed for people who have disabilities other than visual or hearing disabilities. It consists of four parts about you: health, demographics, daily activities, and experiences with Service Dogs* or pet dogs if you have them. This questionnaire is not limited to people living with dogs. Non-dog owners can also take this survey.
